Job summary

Community Care Live is seeking an experienced Adult Learning Disabilities Social Worker to join a team in Coventry, delivering person-centred support to adults with learning disabilities and safeguarding. You will assess needs, develop support plans and work with families and other professionals to promote independence.

The contract is full-time and on-going, paying £32 per hour. Benefits include dedicated consultant, DBS and compliance support, and access to a wide range of social work

Qualifications

• Experience as a Social Worker in adult services, with a focus on learning disabilities.
• Strong safeguarding knowledge and risk assessment skills.
• Ability to develop person-centred support plans with service users and families.

Responsibilities

• Manage caseload of adults with learning disabilities; conduct assessments and create person-centred support plans.
• Carry out Care Act assessments and reviews per legislation and local procedures.
• Promote independence, choice and positive outcomes for service users.
• Assess and manage safeguarding concerns; address risks appropriately.
• Collaborate with families, carers, healthcare professionals and multi-agency teams.
• Support individuals through transitions and changing care needs.
• Maintain accurate records and comply with statutory requirements and professional standards.
