Midlands Partnership NHS Foundation Trust is recruiting to deliver integrated, strengths-based interventions for adults with complex needs and substance use concerns. You will work in community settings and make home visits to support access to services and work-related activities.
You will collaborate with social workers and partners to carry out assessments, risk reviews and develop person-centred support plans, while safeguarding client information with compliance to policy.

Working alongside adult social care, you will be responsible for the delivery of integrated, accessible and strengths-based interventions for adults with complex needs and substance use concerns. You will need to focus on safeguarding, risk management and positive outcomes with the aim of reducing the need for costed adult social care packages.
Hold a caseload of clients with multiple needs. You will attend and contribute to multi agency meetings. You will also be required to update client records in a timely manner and complete formal reports. You will provide therapeutic and outreach support, including home visits and community-based work, and supporting access to work-related activities. You will work closely with social workers and multi-agency partners to carry out assessments, risk assessments, reviews and complete support plans, ensuring a person-centred approach.
