Home Health (Visits) Admissions Physical Therapist

BAYADA Home Health has an exciting opportunity for a Full-Time Admissions Physical Therapist. You will complete OASIS Starts of Care (SOCs) for adult and geriatric patients in Union, Middlesex, Monmouth, and/or Somerset Counties. One year of prior clinical experience as a licensed Physical Therapist is required; Home Health and OASIS experience are strongly preferred. As a home care Physical Therapist, you will be an integral member of a multidisciplinary healthcare team providing rehabilitative care and skilled nursing to clients, enabling them to remain at home.

Responsibilities include:

• Making home visits to clients in designated geographic areas.
• Performing diagnostic tests and measurements, including mobility, joint range of motion, transfer status, stability, ambulation patterns, muscle strength and endurance, balance, and safety assessments.
• Developing and implementing individualized care plans with manual therapeutic exercises, gait training, balance, and other interventions.
• Assessing and revising care plans regularly, collaborating with nursing staff as needed.
• Educating clients and their families on rehabilitative activities to promote health, safety, and independence.
• Documenting observations, interventions, and evaluations accurately using a tablet device.

Qualifications:

• Current New Jersey Physical Therapist License.
• Minimum of one year recent work experience as a Physical Therapist.

Additional Information:

Base pay depending on qualifications: $60.00 - $65.00 per hour
