A community-focused healthcare provider in Morpeth is seeking to support its healthcare team within a prison environment. The role involves coordinating care and liaising with various agencies as part of a multidisciplinary team. This role is to support our integrated healthcare team in the delivery and monitoring of care. The successful candidate will be required to coordinate their work within the prison environment as well as liaising with outside agencies and will be part of a multidisciplinary team.
