Job Summary
The Acute Referral Triage Nurse is a key role within the Urgent & Emergency Care Collaborative and is a fundamental part of the Multi Agency Telephone Coordination Hub (MATCH).MATCH is a newly implemented single point of access service for acute medical referrals. Job Summary

The post holder will lead the triage and streaming process for all urgent and emergency care referrals into the Acute Trust. The post holder will ensure patients are directed to the right place via the most appropriate clinical pathway, based on clinical assessment and referral information.
Main duties of the job
This role involves autonomous advanced decision-making, critical thinking, multidisciplinary collaboration, and service improvement initiatives. The Acute Referral Triage Nurse will be pivotal in optimising patient flow, improving clinical outcomes and supporting admission avoidance by ensuring we are safely and effectively managing the right patients in the right place. Strong leadership, advanced clinical assessment and triage skills and experience in acute care are essential.
