As part of a multi-disciplinary team you will assess, plan, implement and evaluate high-quality, evidence-based nursing care to prevent hospital admission and support rapid discharge.

Qualifications

• Substantial post registration in Acute or Community Team (from job content)
• Experience of a wide range of medical conditions
• Evidence of good medical grounding within an acute setting
