Job overview

As part of the NHS Community Mental Health Transformation Programme, an exciting opportunity has arisen for a Band 7 Mental Health Nurse Practitioner to work as part of the Community Urgent Care Response Teams in Primary Care, offering specialist Mental Health support to patients cared for within the community. The post holder will work closely with Physical Health Urgent Care Team colleagues in Primary Care, providing advice and support for patients with complex or challenging Mental Health needs. Where appropriate, they will provide initial assessment and first-line treatment, followed by signposting, including direct access to support from Older Adults Enhanced Community teams. The post holder will ensure high standards of assessment, including risk management and treatment for patients referred to the service, including those with complex Mental Health needs.

Main duties of the job

The post holder will exercise a high degree of professional autonomy and critical judgment, making autonomous decisions based on knowledge, skills, and experience, and maintaining competencies through supervision. They will provide specialist Mental Health advice and guidance to health professionals, care homes, and patients within Physical Health Virtual Wards, including initial assessments and first-line treatment. They will also provide education and training to professionals, patients, families, and carers about Older Adult Mental Health, aiming to improve health outcomes and deliver care closer to home. Additionally, they will undertake Independent Non-Medical Prescribing where appropriate, utilizing NICE Guidance and Trust policies, or be willing to undertake NMP training.

Detailed job description and main responsibilities

  • Undertake Independent Non-Medical Prescribing where appropriate, following NICE Guidance and Trust policies.
  • Work closely with Physical Health Urgent Care Team colleagues, providing advice and support for patients with complex mental health needs, including initial assessments and treatment, with signposting and direct access to Older Adults Enhanced Community Mental Health Teams.
  • Support care planning for individual patients and provide education and practice development for referrers.
  • Provide education and training for professionals, patients, families, and carers to support older adult mental health and improve health outcomes.
  • Provide clinical leadership, offering specialist mental health advice and guidance, initiating change based on audit and research.
  • Exercise professional autonomy, making decisions based on knowledge and experience, maintaining competencies through supervision.
  • Conduct risk and other assessments, documenting them promptly according to Trust policies.
  • Develop links within the Trust and wider community, including social care and voluntary sectors, to meet patient needs during referral, assessment, and signposting phases.
