Guildhall Surgery in Folkestone is seeking an experienced ACP or ANP for a full-time, permanent role within our forward-thinking GP practice. The post combines clinical triage and long-term condition management to improve patient care.

You will work closely with GPs and a multidisciplinary team, provide face-to-face and remote consultations, and contribute to service development while maintaining high standards of patient safety.

Competenze

• Qualified ACP or ANP with registration in good standing.
• Experience in primary care settings and long-term condition management.
• Ability to work autonomously and collaboratively within a GP practice.

Mansioni

• Carry out clinical triage using established workflows and direct patients to appropriate care.
• Provide clinical consultations both face-to-face and remotely.
• Manage and review patients with long-term conditions, creating and implementing care plans.
• Collaborate with GPs, practice nurses, pharmacists and other team members to deliver integrated care.
