Job description

Job Title: Accommodation Pathways Coordinator

Reports to: Youth Services Manager

Salary: Scale 6 – Point 26-28, £26,229 - £27,982 per annum, pro rata

Hours of work: Full Time (37.5 hours per week)

*Fixed Term Contract for 6 months*

Key Responsibilities:
  1. Receive and respond to referrals from social workers, personal advisers, and placement officers in a timely manner.
  2. Undertake strengths-based, person-centred housing and support needs and risk assessments.
  3. Plan responses to presenting needs and circumstances in partnership with young people, parents/carers, other agencies, and professionals to prevent homelessness, identify, and negotiate the most suitable housing options.
  4. Work within the resources provided by Birmingham Children’s Trust, Commissioning Team, or Vulnerable Adults provision.
  5. Ensure placements and services identified through risk management plans meet the needs of young people, considering health, educational needs, ability, gender, religion, ethnic origin, culture, and linguistic background.
  6. Provide professional advice, information, and support to stakeholders such as social workers, heads of service, and youth offending/probation teams regarding services available for housing and support.
  7. Manage and prioritize own caseload, evaluate cases, make decisions on appropriate options, meet deadlines, and refer to other services and agencies as appropriate.
  8. Represent St Basils at meetings such as monthly support provider meetings, contract meetings, and case reviews regarding young people.
  9. Ensure the service is accessible to young people, keep all relevant parties informed of outcomes, and provide follow-up support as needed.
  10. Stay updated with legislation, regulations, and developments related to young people and housing resources.
  11. Assist in service development, achieving key performance indicators, and updating policies and procedures.
  12. Maintain statistical information for monitoring and evaluation, produce reports for stakeholders such as Birmingham Children’s Trust.
  13. Liaise with key partners and funders as appropriate.
  14. Represent and promote the agency’s work at internal and external meetings and forums.
  15. Deliver training and briefings as required.
  16. Participate in induction of new staff, supervise and support volunteers, trainees, and students.
  17. Collaborate with organizations and agencies for holistic multi-agency responses to housing and support needs, e.g., DWP, FTB.
Role Experience:
  • Experience working with young people in settings such as youth work, residential or social work, probation, community work, education, training, or housing.
  • Experience delivering services through payment by outcomes (PBO or PBR).
  • Experience working with young people with complex needs.
  • Experience safeguarding vulnerable children, young people, or adults.
Qualifications:
  • Youth Work, Social Work, or equivalent qualification.
  • NVQ Level 4 in Advice & Guidance, Care, or equivalent, or working towards it.
