Job Title: Accommodation Pathways Coordinator
Reports to: Youth Services Manager
Salary: Scale 6 – Point 26-28, £26,229 - £27,982 per annum, pro rata
Hours of work: Full Time (37.5 hours per week)
*Fixed Term Contract for 6 months*
Key Responsibilities:
- Receive and respond to referrals from social workers, personal advisers, and placement officers in a timely manner.
- Undertake strengths-based, person-centred housing and support needs and risk assessments.
- Plan responses to presenting needs and circumstances in partnership with young people, parents/carers, other agencies, and professionals to prevent homelessness, identify, and negotiate the most suitable housing options.
- Work within the resources provided by Birmingham Children’s Trust, Commissioning Team, or Vulnerable Adults provision.
- Ensure placements and services identified through risk management plans meet the needs of young people, considering health, educational needs, ability, gender, religion, ethnic origin, culture, and linguistic background.
- Provide professional advice, information, and support to stakeholders such as social workers, heads of service, and youth offending/probation teams regarding services available for housing and support.
- Manage and prioritize own caseload, evaluate cases, make decisions on appropriate options, meet deadlines, and refer to other services and agencies as appropriate.
- Represent St Basils at meetings such as monthly support provider meetings, contract meetings, and case reviews regarding young people.
- Ensure the service is accessible to young people, keep all relevant parties informed of outcomes, and provide follow-up support as needed.
- Stay updated with legislation, regulations, and developments related to young people and housing resources.
- Assist in service development, achieving key performance indicators, and updating policies and procedures.
- Maintain statistical information for monitoring and evaluation, produce reports for stakeholders such as Birmingham Children’s Trust.
- Liaise with key partners and funders as appropriate.
- Represent and promote the agency’s work at internal and external meetings and forums.
- Deliver training and briefings as required.
- Participate in induction of new staff, supervise and support volunteers, trainees, and students.
- Collaborate with organizations and agencies for holistic multi-agency responses to housing and support needs, e.g., DWP, FTB.
Role Experience:
- Experience working with young people in settings such as youth work, residential or social work, probation, community work, education, training, or housing.
- Experience delivering services through payment by outcomes (PBO or PBR).
- Experience working with young people with complex needs.
- Experience safeguarding vulnerable children, young people, or adults.
Qualifications:
- Youth Work, Social Work, or equivalent qualification.
- NVQ Level 4 in Advice & Guidance, Care, or equivalent, or working towards it.
