This permanent role involves providing assessment and rehabilitation support to individuals at home, assisting with hospital discharges, and collaborating with healthcare professionals. Community Rehabilitation – Hub Therapy Team
An exciting opportunity has arisen to join the Hub Therapy Team within HomeFirst, Community Rehabilitation and Reablement Services in Edinburgh Health and Social Care Partnership. The therapy team provides assessment and rehabilitation to support people at home who are at risk of hospital admission and facilitates hospital discharge for people who require increased support on their initial discharge from hospital. The post holder will support Occupational Therapists, Physiotherapists and Health Care Support Workers by taking an active clinical and administrative role within Hub Therapy, as delegated and guided by the registered practitioner. The post holder will have a designated base, with the expectation to work and provide admin support for the team city wide. There are several healthcare support workers within the team, with a strong supervision and support structure in place. You will be offered regular training opportunities and you will attend a yearly appraisal to focus on your development goals.
This post requires the post holder to have a PVG Scheme membership/ record. If the post holder is not a current PVG member for the required regulatory group (i.e. child and/or adult) then an application will need to be made to Disclosure Scotland and deemed satisfactory before they can begin in post.
