Job summary

Responsibilities
- Act as a full member of the staff team of the Service and contribute to its development within the Directorate.
- Manage an individual caseload, work with young people and groups, participate in duty work, and respond to needs as they arise.
- Participate in providing appropriate programmes for service users, aiding their transition to adulthood, especially from being in care to independent living.
- Implement and adhere to all relevant Directorate and Service procedures as an aftercare adviser.
- Adopt a holistic approach to meet individual needs, promote involvement, and develop self-esteem through positive achievements.
- Advise other staff on the preparation and support needs of care leavers and promote related services.
- Liaise with Directorate and external agencies to maximize resource provision for service users.
- Provide information and support to young people individually or in groups.
- Work directly with assigned young people, ensuring assessment, intervention, and record-keeping are completed.
- Participate in duty/referral services, advising young people and referrers.
Minimum Requirements
- Stay updated on trends and developments in legislation, policy, and practice related to aftercare, accommodation, employment, benefits, and childcare.
- Undertake appropriate training, including NVQ level 3, to develop skills and services.
- Participate in supervision sessions and working parties to improve 18+ services.
- Contribute to training programmes for staff and partner agencies.
- Work flexible hours as needed.
- Possess substantial experience working with children and young people up to age 21 or 25 in higher education, in voluntary or professional settings.
- Experience in interagency work with vulnerable, at-risk, homeless, or former care young people.
- Experience in directly assisting young people with caseload responsibilities.
- Proven success in achieving outcomes for young people under supervision.
