An exciting job role has recently become available in Birmingham. They are looking for a new member for their Adult Constituency teams. This role offers a total of £31.48 for the contract duration.

Role Overview
  • Support vulnerable adults in the community
  • Promote independence, well-being, and social inclusion
  • Collaborate with individuals, families, and professionals
  • Drive positive outcomes through compassionate approach and strong assessment skills
Key Responsibilities
  • Conduct comprehensive assessments of individuals’ needs
  • Develop personalized support plans
  • Coordinate services with health professionals, care providers, and community resources
Safeguarding and Risk Management
  • Identify and respond to safeguarding concerns promptly
  • Assess risks related to vulnerable adults
  • Ensure legal compliance and safety of service users
Direct Work with Clients
  • Provide emotional support, counseling, and advocacy
  • Assist in accessing services, benefits, and housing
  • Facilitate group sessions and workshops to enhance social skills
Collaboration and Multi-Agency Working
  • Liaise with healthcare professionals, housing officers, police, etc.
  • Attend case conferences and multidisciplinary meetings
  • Advocate for clients’ rights and needs within the system
Documentation and Record Keeping
  • Maintain accurate and up-to-date case records
  • Complete assessments, care plans, and reports timely
  • Ensure compliance with data protection and confidentiality guidelines
Qualifications and Experience
  • Degree in Social Work or equivalent
  • Registration with Social Work England
  • Experience in adult social care
  • Knowledge of relevant legislation (e.g., Care Act 2014, Mental Capacity Act 2005)
Additional Information
