0-19 Service - Specialist Public Health Practitioners - Health Visitors / School Nursing Band 6
The 0 to 19 enhanced pathway is an integral and distinct caseload within the Croydon 0 to 19 Public Health Nursing (PHN) service. A programme of care will be delivered to families and young people with additional health needs and vulnerabilities determined by inclusion criteria.
The aim is to improve health and development outcomes of babies, children and young people and their families. The tools that will be used by the Enhanced Pathway Nurses for example the Family Partnership Model and Outcome Star are underpinned by research to get the best outcomes for children, young people and families.
We are actively searching for qualified Health Visitors and School Nurses who are eager to enhance their leadership capabilities and contribute to the future direction of Public Health Nursing in Croydon. We're looking for individuals who are enthusiastic about collaborative work, possess warmth and excellent communication skills, and demonstrate a solid grasp of the Healthy Child Programme and Safeguarding policies. Our dynamic learning and development team promises an exciting journey within our service.
This role is an integral part of a multidisciplinary team dedicated to delivering the Healthy Child Programme and offering additional support to families with children aged 0-5. Our aim is to implement an integrated approach to Health Visiting throughout Croydon Health Services.
