Band 5 Community Staff Nurse – 0-19 Service

Are you passionate about making a difference in the lives of children, young people, and families? We are seeking a dedicated and compassionate Band 5 Community Staff Nurse to join our dynamic 0-19 team.

As part of the wider community nursing and primary healthcare team, you will work under the guidance of the Specialist Community Public Health Nurse (Team Lead), delivering services including health surveillance, health promotion, immunisations, and providing support and advice to families with children and young people aged 0-19.

You will collaborate closely with statutory and voluntary partner agencies to achieve optimal health outcomes for children and families. Using evidence-based practice, you will contribute to delivering a safe, effective, and high-quality service aligned with professional standards.

This role involves supporting the implementation of the 0-19 Healthy Child Programme within a progressive universal model, tailoring interventions based on assessed needs, and ensuring seamless partnership working across agencies.

To support the delivery of the 0-19 Healthy Child Programme (HCP) in providing an innovative and high-quality service that helps children, young people, and families thrive, the post holder will work independently to support clients, with access to support, advice, and management from a SCPHN clinician.

The post holder will be responsible for their workload within their specialty, assessing, planning, delivering, and evaluating patient-centered care, practicing within their competence, and adhering to professional protocols and standards grounded in evidence-based knowledge.

Key Responsibilities
  1. Assess, plan, implement, and evaluate individual and family-focused care under the delegation of the SCPHN and in partnership with other agencies.
  2. Establish and maintain partnerships with families and other agencies in a multi-agency environment.
  3. Participate in locality meetings.
  4. Maintain a professional service image and act as a role model for staff.
