Job description

The post holder will be a member of the 0-19 service and will undertake duties as delegated by the Specialist Community Public Health Nurse (Team Lead). The post holder will provide a service which includes health surveillance, health promotion, support and advice, and immunisations primarily to families with children under 0-19 years of age. The post holder will work as part of the wider community nursing and primary health care team, liaising and working with other statutory and voluntary agencies as appropriate.

0-19 teams provide a safe, effective and high-quality service to clients using research-based evidence to inform practice within agreed professional standards. The post holder will work in collaboration with partner agencies, both voluntary and statutory, to achieve good health outcomes for children and their families.

The Band 5 Community Staff Nurse will support the delivery of the 0-19 Healthy Child Programme within a progressive universal model of service delivery depending on assessed needs, ensuring the programme is delivered in partnership with other statutory and non-statutory agencies. This role will play a key part in the development and safe delivery of 0-19 services.

To support the delivery of the 0-19 Healthy Child Programme (HCP) in providing an innovative and high-quality service that helps children, young people, and families to thrive, working without direct supervision to support clients.

The post holder will be responsible for their designated workload within their specialty area with access to the support, advice, and management from a Specialist Community Public Health Nurse (SCPHN) clinician whilst not directly supervised. In doing so, they will assess, plan, deliver, and evaluate patient-centered programmes of care ensuring that they practice within their sphere of competence and knowledge. The post holder will work to professional protocols and regulatory body codes, standards, and guidance at all times ensuring that their practice is grounded in the best available evidence-based theoretical and practical knowledge.

Key Responsibilities:

1. Assess, plan, implement, and evaluate individual, family-focused care, under the delegation of the Specialist Community Public Health Nurse (SCPHN) (Health Visitor/School Health Nurse) and working in partnership with other agencies.
2. Establish and maintain partnerships with families and other agencies in a multi-agency and integrated working environment.
3. Attend and participate in locality meetings.
4. Ensure that a professional service and image is maintained at all times, acting as a role model for staff.
