Job summary

The job holder supports Registered Nurses / Enrolled Nurses and senior care staff in delivering safe, person-centred home care and Enhanced Home Personal Care services to clients. The role includes personal care, ADL assistance, basic monitoring, home and IADL support, and escalation of concerns in the home setting.

Qualifications

• Completed Nursing Diploma or equivalent healthcare qualification.
• Ability to understand and speak English.
• Experience in home care, nursing home, hospital, senior or community care preferred.

Responsibilities

• Provide personal care, ADL assistance and HPC+ support per care plan.
• Assist with ADLs such as bathing, grooming, dressing, toileting and continence care.
• Support oral feeding and safe feeding tasks as trained, excluding tube insertion.
• Assist with safe lifting, transfers, positioning and prescribed exercises.
• Provide companionship, cognitive stimulation and elder-sitting as planned.
• Provide medication reminders or assistance as trained and authorized.
• Monitor basic observations; escalate changes to nurses/supervisors.
• Ensure infection prevention and safety boundaries are observed.
• Support HPC+ housekeeping and IADL tasks per care plan.
• Document observations accurately and timely; report issues to care team.
• Precept and guide new HCAs during onboarding and practice.
