Responsibilities
- Assess and monitor patients’ health conditions in their home environment.
- Develop and implement individualized care plans.
- Educate patients and families on health management and self-care.
- Collaborate with a multidisciplinary team to ensure optimal patient outcomes.
- Provide patient care using the nursing process (assess, plan, implement, evaluate) for patients across developmental stages.
- Coordinate care with a multidisciplinary team and manage a caseload effectively.
- Communicate information in accordance with agency policies and guidelines.
- Contribute to program effectiveness and promote customer satisfaction.
- Demonstrate the mission, vision, and values of the agency daily.
Job Specifications / Qualifications
- Education: Graduate of an accredited nursing program (AD, Diploma, or BSN).
- Licensure / Certifications: Current nursing license in the state of practice (NC/SC). CPR certification required.
- Experience: One year of RN experience; total of 2+ years of clinical experience. Home health experience preferred. If RN experience is less than 1 year, 1 year of clinical experience as an LPN may be required (internal use only).
- Technical / Motor Skills: Proficient in English; able to use computers and maneuver patient care equipment.
- Interpersonal Skills: Ability to develop positive interactions with patients, families, physicians, and staff.
- Physical Requirements: Ability to transfer and move patients and objects; frequent lifting and assistance in patient care activities.
- Mental and Sensory Abilities: Ability to assess conditions, plan and evaluate care, prioritize duties, and communicate effectively.
- Work Environment: Hours vary; flexible schedule including weekends and holidays as needed. May involve exposure to hazards and temperature changes.
- Population Served: Adolescents, adults, geriatrics, and pediatrics.
- Driver Requirement: Must have a valid driver’s license and reliable transportation.
