Well Care Health is seeking a compassionate Registered Nurse to provide home health care and support to patients in their homes in Nouvelle-Calédonie. The role focuses on delivering high-quality patient care within a supportive environment.

You will assess health conditions, develop care plans, educate patients and families, and collaborate with a multidisciplinary team to achieve optimal outcomes while adhering to agency guidelines and values.

Qualifications

• Graduate of an accredited nursing program (AD, Diploma, or BSN).
• Current RN license in the state of practice; CPR certification required.
• Minimum one year RN experience with home health preferred.
• Proficient in English; able to operate computers and maneuver patient care equipment.

Responsibilities

• Assess and monitor patients’ health conditions in their home environment.
• Develop and implement individualized care plans.
• Educate patients and families on health management and self-care.
• Collaborate with a multidisciplinary team to ensure optimal patient outcomes.
• Provide patient care using the nursing process (assess, plan, implement, evaluate) for patients across developmental stages.
• Coordinate care with a multidisciplinary team and manage a caseload effectively.
• Communicate information in accordance with agency policies and guidelines.
• Contribute to program effectiveness and promote customer satisfaction.
• Demonstrate the mission, vision, and values of the agency daily.

Job Specifications / Qualifications

1. Education: Graduate of an accredited nursing program (AD, Diploma, or BSN).
2. Licensure / Certifications: Current nursing license in the state of practice (NC/SC). CPR certification required.
3. Experience: One year of RN experience; total of 2+ years of clinical experience. Home health experience preferred. If RN experience is less than 1 year, 1 year of clinical experience as an LPN may be required (internal use only).
4. Technical / Motor Skills: Proficient in English; able to use computers and maneuver patient care equipment.
5. Interpersonal Skills: Ability to develop positive interactions with patients, families, physicians, and staff.
6. Physical Requirements: Ability to transfer and move patients and objects; frequent lifting and assistance in patient care activities.
7. Mental and Sensory Abilities: Ability to assess conditions, plan and evaluate care, prioritize duties, and communicate effectively.
8. Work Environment: Hours vary; flexible schedule including weekends and holidays as needed. May involve exposure to hazards and temperature changes.
9. Population Served: Adolescents, adults, geriatrics, and pediatrics.
10. Driver Requirement: Must have a valid driver’s license and reliable transportation.
