Qualifications

• Valid driver's license with a clean MVR.
• Medication certification within the provisional period.

Responsabilités

Description

Position Summary:

The position is responsible to assist individuals in identifying and/or acquiring skills to be as independent as possible and establish an environment that provides safety and skill enhancement.

Primary Duties & Essential Functions:

Care of Individuals

1. 1. Employs procedures that maintain an environment of care that is safe and therapeutic.
2. 2. Provides direct care to individuals per agency policy.
3. 3. Implements appropriate supervision to assigned daily schedules, including all routines and life skills activities.
4. 4. Dispenses medication per agency policy.
5. 5. Safely transports individuals as assigned per level of supervision required.
6. 6. Assists assigned individuals with specific tasks as designated by individual treatment plan.
7. 7. Participates in the treatment team or recommendation process.
8. 8. Assists in personal care of individuals.
9. 9. Assures appropriate boundaries

Policies and Procedures

1. 1. Learns and practices agency policies and procedures.
2. 2. Learns and implements each individual's treatment plan, including physical interventions, documents, and makes proper notifications according to procedures.
3. 3. Provides coverage as assigned.
4. 4. Remains on duty until properly briefed and appropriate parameters are met.

Supervision

1. 1. Actively participates in supervision.
2. 2. Participates in agency and other staff development opportunities as assigned.
3. 3. Attends and actively participates in all assigned trainings.

Agency Responsibilities

1. 1. Engages in all living unit, service, and/or department planning and goal attainment.
2. 2. Participates in activities and committees.
3. 3. Changes work location to meet agency needs.
4. 4. Maintains appropriate attendance and punctuality.

Communication

1. 1. Reviews and signs off on all logs and thoroughly completes briefing.
2. 2. Completes all necessary communications, documentation, and logging.
3. 3. Properly notifies and/or consults with the Administrator on Duty (AOD).
4. 4. Proactively and effectively liaises with other disciplines, internal and external Service Providers, and families as appropriate.
5. 5. Appropriately utilizes the chain of command.
6. 6. As assigned, will complete section of monthly report.

Environment of Care

1. 1. Completes assigned cleanup procedures, checklists, and schedules.
2. 2. Proactively and effectively liaises with Maintenance, House Manager, Coordinators, Direct Support Supervisors and Directors.
3. 3. Runs routine in the absence of other team members when necessary
4. 4. Supports Quality Assurance Quality Improvement directives.

Other Responsibilities

1. 1. Completes other responsibilities as properly assigned. When required, assures and assists with supervision of all individuals served.
2. 2. Promotes and supports cultural diversity and competence and is strength-based.

Required Education, Knowledge and Skills:

Pay: $20+ an hour based on education and experience.
