Providence at Home with Compassus is hiring a Home Health LPN/LVN to deliver holistic nursing care under RN supervision across assigned territories. You will support plans of care, monitor patient conditions, and communicate effectively with patients and families during end-of-life care and chronic illness management.

We seek graduates of an accredited LPN/LVN program with 1 year of relevant experience; proficiency with EHR and MS Office is preferred.

Competenze

• Graduate of an accredited Licensed Practical/Vocational Nurse program.
• Preferred 1 year experience in home health, hospital, or chronically ill patients.

Mansioni

• Provides LPN/LVN level nursing services as established by the RN.
• Observes, implements nursing interventions and provides data for the Plan of Care.
• Implements nursing interventions based on the Plan of Care and policies.
• Anticipates disease progression and implications for symptom management.
• Instructs patient and family in all aspects of care and end-of-life as well as disease progression and symptom management.
• Manages assigned caseload and communicates changes in Plan of Care to RN as appropriate.
• Documents observations, interventions, and communications accurately and timely.
• Participates in continuous quality improvement activities and supports team function.
