Job summary

Kanta-Hämeen hyvinvointialue hakee hoiva-avustajaa Riihimäen arviointi- ja kuntoutusyksikköön. Yksikkö tarjoaa lyhytkestoista ympärivuorokautista hoitoa potilaan kotiinpaluun tueksi sairaalahoidon jälkeen, moniammatillisen tiimin ohjauksessa.

Työn aloitus sovittavissa, viimeistään 1.10.2026. Tehtävä tarjoaa mahdollisuuden tukea potilaiden kuntoutusta, osallistua kotihoitoon ohjaukseen ja toteuttaa päivittäisiä hoitotoimenpiteitä osana tiimiä.

Qualifications

• Hoiva-avustajan tutkinto (vaatimus)
• Kokemus hoitotyöstä (toivottavaa)
• Kokemus arviointi- ja/tai kuntoutusyksiköstä (toivottavaa)

Responsibilities

• Osallistutaan potilaiden hoitoon ja kuntoutukseen moniammatillisen tiimin osana.
• Osallistutaan kotihoitoon ohjaukseen ja päivittäisten hoitotoimenpiteiden toteuttamiseen potilaiden turvallisuuden ja hyvinvoinnin edistämiseksi.

We are hiring a permanent care assistant for Riihimäki Assessment and Rehabilitation Unit. Start date by agreement, or at the latest 1 October 2026.

The Riihimäki unit provides short-term, around-the-clock care when home living is not yet possible after hospital care. It offers round-the-clock assisted living with housing and services for customers needing extensive care, attention and monitoring at all times.

The unit employs a multidisciplinary team including nurses, practical and assistant nurses, care assistants, physiotherapists, a discharge coordinator, and housekeeping services.

Doctor services are provided by Oma Häme and there is an on-call doctor and gerbil.

The aim of the temporary assessment and rehabilitation unit is to activate and improve the customer’s functional capacity and resources so they can be guided to the right place, such as home care or another service.

During the assessment and rehabilitation period, the customer takes responsibility for their own rehabilitation under the multidisciplinary team guidance.

The team, together with the customer and their relatives, sets objectives for the period to which the customer and unit commit.
