Responsibilities

• Perform comprehensive medical (geriatric) assessments in collaboration with a multidisciplinary team.
• Assessments are always conducted at the patient’s home; there are no separate consultation rooms.
• Doctors perform extensive initial assessments and meet patients during periodic visits.
• Focus on enabling home-based living for patients, evaluating medication benefits and risks, fall risk and cognitive impairment management.
