Amae Health is seeking a Clinical Care Coordinator to support members across our two Northern California clinics in San Mateo and Los Altos. You will help create a warm, engaging environment, facilitate group activities, and assist with daily workflows while collaborating with clinicians and staff.

The role emphasizes empathy, teamwork, and growth in mental health support, with opportunities to learn and contribute to a high-acuity care model.

Formación

• Enrolled or recently graduated in a degree program related to mental health, such as pre-med, psychology, social work, or nursing
• Interest in mental health and desire to work with members with Severe Mental Illness
• Strong interpersonal skills and ability to work in a team environment
• Detail-oriented and organized
• Comfortable working either one-on-one or in group settings, must possess excellent interpersonal skills and be able to adapt to changing situations
• Willingness to collaborate with others
• Well-organized and some experience planning and designing events and activities
• Basic computer skills are required
• The candidate should possess a holistic perspective of health.

Responsabilidades

• Facilitate groups, arts and crafts, and recreational activities to enhance the member experience.
• Welcome members to the clinic, manage communications via phone and email, and assist with scheduling and member record updates to support smooth daily operations.
• Collaborate with the interdisciplinary care team - psychiatrists, therapists, dietitians, and primary care providers - to share observations and support coordinated member care.
• Identify members' ongoing needs and help connect them with community resources and supports that promote wellness.
• Contribute to projects that support clinic growth and ongoing member success. Participate in training and supervision to build skills in mental health support, community resources, and member engagement.
