Praxis Health seeks a Quality Improvement Manager to lead HEDIS performance, care-gap closures, and payer incentive initiatives. You will partner with Population Health leaders and operate as the EMR quality expert, optimizing workflows and data capture to drive quality outcomes.

The role requires educating staff on quality measures and fostering a culture of continuous improvement across clinical and operational teams.

The Quality Improvement Manager is responsible for leading and advancing organizational quality performance with a primary focus on HEDIS measures, payer incentive programs, and population health initiatives. This role oversees the identification of quality improvement opportunities, monitors performance across multiple payer programs, develops strategies to improve outcomes, and leads a team responsible for supporting quality and performance initiatives.

The Quality Improvement Manager partners closely with the Director of Population Health and other clinical and operational leaders to develop and implement sustainable quality improvement strategies. This individual serves as the organization’s subject matter expert for quality within the electronic medical record (EMR), ensuring that clinical workflows, documentation, data capture, and EMR functionality effectively support quality performance.

Key Responsibilities
Quality & HEDIS Performance
  • Lead the organization’s HEDIS performance strategy, including monitoring, analysis, gap identification, and improvement initiatives.
  • Track and analyze performance across HEDIS measures and other quality metrics to identify trends, barriers, and opportunities for improvement.
  • Develop and implement targeted interventions designed to close care gaps and improve clinical outcomes.
  • Monitor progress toward organizational, payer, and provider-level quality goals.
  • Ensure quality initiatives are aligned with regulatory requirements, payer expectations, and organizational priorities.
  • Translate quality data into actionable insights for clinical and operational teams.
Team Leadership & Management
  • Lead, manage, and develop a team supporting HEDIS, quality improvement, care gap closure, and payer incentive initiatives.
  • Establish clear goals, workflows, priorities, and performance expectations for the quality team.
  • Monitor team productivity and effectiveness and identify opportunities to improve processes.
EMR Quality Expertise
  • Serve as the organization’s subject matter expert on quality-related functionality within the EMR.
  • Develop deep expertise in how quality measures, care gaps, clinical documentation, workflows, health maintenance, registries, and reporting are represented within the EMR.
  • Identify opportunities to optimize EMR workflows and functionality to support quality performance.
Education & Staff Engagement
  • Develop and deliver education and training related to HEDIS measures, quality requirements, payer programs, documentation, and care gap closure.
  • Create educational materials, tools, and resources to support ongoing quality improvement.
  • Promote organizational awareness and accountability for quality performance.

General Physical Requirements
