Premier Home Health Providers Inc. is seeking an Occupational Therapist to provide per diem home health services. You will evaluate patients in their homes, develop and adjust treatment plans, and guide patients in daily living activities to regain independence.

Candidates must hold an OT degree, be licensed in California, and be registered to practice. A minimum of two years of OT experience, CPR certification, and the ability to travel are required.

Formación

• Graduate of an OT curriculum accredited by the American Medical Association and American Occupational Therapy Association.
• Registered with the National Registration Examination of the American Occupational Therapy Association.
• Active California Occupational Therapy license.
• Minimum 2 years of clinical OT experience.
• Current CPR certification with AHA.
• Valid driver’s license and ability to travel to patient homes.
• Strong communication and organizational skills.

Responsabilidades

• Evaluate patients’ functional abilities and home environments to identify needs and safety concerns.
• Develop personalized treatment plans and communicate them to physicians and the care team.
• Teach and retrain patients in daily living skills affected by illness or injury.
• Recommend and instruct patients on adaptive equipment, splints, and prosthetic/orthotic devices.
• Maintain accurate clinical records and promptly report changes in patient condition.
• Coordinate with the broader care team to ensure patient-centered care.
• Educate family members and caregivers on the treatment approach.
• Supervise OT Assistants and oversee home health aide care when OT is the sole skilled service.
• Support discharge planning as patients transition out of home health care.
• Perform other duties as required for patient services.
