At Tertianum Residence St. Jakob Park in Basel, we are looking for an immediately available nurse HF/FH/DNII or SRK-recognised professional to join our caring team.

You will provide holistic care to our guests, lead nursing processes, and collaborate with multidisciplinary teams to maintain high standards of nursing quality. You will work with RAI LTCF expertise, ensure proper documentation with CareCoach, and support colleagues in daily practice while promoting person-centred care and ongoing

Qualifications

• Qualified nurse HF/FH/DNII or SRK-recognised equivalent.
• Health Specialist EFZ qualification.
• Experience in long-term care is an advantage.

Responsibilities

• Provide holistic care and nursing of guests per nursing process.
• Serve as primary nursing contact for assigned guests.
• Manage nursing process including RAI/BESA assessments.
• Use CareCoach for electronic nursing documentation.
• Plan and accompany medical visits.
• Coordinate day team and lead case discussions.
• Support staff on RAI LTCF and nursing quality.

Your tasks

• Holistic care and nursing of guests in accordance with our nursing philosophy and nursing process
• Assuming tasks as the primary nursing contact person
• Responsibility and management of the nursing process for assigned guests, including RAI/BESA nursing needs assessment
• Electronic nursing documentation using CareCoach
• Planning and accompanying medical visits
• Assuming daily responsibility with coordination of the day team
• Leading case discussions and nursing reports
• Assuming tasks as a topic leader in a specialist area and thus contributing to the group-wide network for nursing quality
• Supporting and advising staff on questions regarding RAI LTCF and the nursing process
• Recognising changes in nursing and care needs and promptly adjusting the corresponding planning
• Participating in case discussions, nursing visits and interprofessional meetings
• Promoting resource-oriented and person-centred nursing
• Active involvement in securing and further developing our nursing quality

Your profile

• Completed training as a qualified nurse HF/FH/DNII or an equivalent qualification recognised by the SRK
• Completed training as a health specialist EFZ
• Professional experience in long-term care is an advantage

• Solid practical experience with RAI LTCF
• Professional experience in Spitex/In-house Spitex desired

• Very good knowledge of the nursing process and professional nursing documentation
• Confidence in conducting and assessing assessmentsStrong quality and sense of responsibility
• Independent, structured and reliable working style
• Enjoyment in sharing expertise and supporting colleagues in daily work
• Appreciative and respectful attitude towards our guests and employees
• Good German language skills, both spoken and written, and confidence in using digital documentation systems
