The Primary Health portfolio at the IWK supports a diverse range of provincial, community based, and operational support programs and services. Emphasis is placed on understanding the health status of the population and the needs of the communities we serve. Primary Health builds relationships and works closely with communities and intersectoral partners to pursue the IWK mission from a policy, system, and upstream approach.
Reporting to the Manager of Primary Health, the Program Facilitator will plan, facilitate, and evaluate the “You’re in Charge” Program. You’re in Charge (YIC) is a pediatric self-management program to help youth with a health condition (age 13 to 15) develop self-management skills and to enable parents/caregivers to initiate discussion leading to more independent self-management in their youth. YIC also works within the IWK, the community and with families to prepare for the transition from pediatric to adult care. The YIC Coordinator works closely with the Transition Coordinator, Children’s Health Program, and supports the Transition of Care committee as well as the Youth Learning Series and the Children’s Health Program orientation.
YIC is a peer-led, youth engagement focused, and evidence-based intervention developed by the IWK Health Centre, the Dalhousie School of Occupational Therapy, Chronic Condition Management Research Group, and by youth who live with health conditions. The program engages a team of trained youth leaders, clinicians, students, and volunteers to deliver various aspects of the program.
Responsibilities include, but are not limited to:
• Plan and coordinate all aspects of program including delivery of weekend retreats and other community-based delivery models
• Recruit and screen program participants and provide follow up support
• Develop and manage program resources and promotional materials
• Oversee program evaluation and program revision
• Develop new adaptations of You`re in Charge for various groups and care teams, including Mental Health
• Contribute to the IWK and NSH transition of care initiatives from pediatric to adult healthcare and the inclusion of youth self-management resources and programming.
