JOB OVERVIEW

The Wellness Nurse is responsible for monitoring the health and well-being of our residents, providing leadership support, and assisting with resident care and services. Responsibilities include, but are not limited to, overseeing the overall well-being of residents, assisting in training and monitoring medication and care administration, communicating with residents, support services, and families, and maintaining clinical quality assurance. The role involves leading and demonstrating the Mission of Sunrise Senior Living to champion the quality of life for all seniors, in accordance with federal, state/provincial, and local standards, as well as Sunrise policies to ensure the highest quality of care and services.

RESPONSIBILITIES & QUALIFICATIONS

Essential Duties

Supporting our Mission Principles of Service and Core Values is fundamental. Responsibilities include but are not limited to:

  • Identifying ongoing needs and services of residents to promote high-quality care.
  • Ensuring proper follow-up, evaluation, and documentation for residents with changes in health status.
  • Assisting the Resident Care Director (RCD) with resident evaluations at move-in, 30 days post-move-in, semi-annually, and with any health changes.
  • Conducting monthly wellness visits for all residents.
  • Monitoring and following up on falls, accidents, or incidents, and documenting accordingly.
  • Maintaining communication with residents’ families or responsible parties.
  • Contacting physicians as needed or upon family request.
  • Ensuring weights and vital signs are obtained monthly.
  • Maintaining wellness files according to policies and regulations.
  • Managing medical supplies and emergency kits.
  • Providing clinical support and assistance to team members.
  • Following infection control practices and promoting standards including resident testing and staff training.

Medication Programs

  • Monitoring residents’ medication and treatment profiles to ensure accuracy and compliance.
  • Transcribing physician orders accurately and promptly.
  • Completing monthly MAR/TAR audits.
  • Demonstrating knowledge of the Six Rights of Medication Pass.
  • Acting as liaison with pharmacy services.
  • Monitoring medication and treatment carts for safety and organization.
  • Completing medication observations and skills checks as directed.

Quality Assurance and Regulatory Compliance

  • Demonstrating knowledge in quality improvement areas such as APIE, Quality Care Indicators, and Outcomes.
  • Participating in Sunrise Quality Assurance and Clinical Meetings.
  • Complying with all relevant regulations, including Resident Rights and assessment processes.
  • Reviewing physician reports and ordering medications and equipment as directed.
  • Training and monitoring documentation related to residents’ health and wellness.
  • Reporting resident concerns or changes to RCD.
  • Engaging in programs to improve team and customer engagement.
  • Attending required meetings and maintaining compliance with training requirements.

Experience and Qualifications

  • Graduate of an approved nursing program.
  • Current state/provincial license as an LPN or LVN.
  • At least one year of experience in a healthcare or long-term care setting.
  • Knowledge of nursing practices, regulations, and assessment skills.
  • Proficiency in computer skills including Microsoft Office and Sunrise applications.
  • Availability to work weekends, evenings, and flexible hours.
