JOB OVERVIEW

The Wellness Nurse is responsible for monitoring the health and well-being of our residents and provides leadership support and assistance with resident care and services. Responsibilities include but are not limited to the overall well-being of our residents, assisting in training and monitoring medication care administration, communication with resident support services and families, and maintaining clinical quality assurance while leading and demonstrating the Mission for Sunrise Senior Living, “to champion the quality of life for all seniors” in accordance with federal, state/provincial, and local standards and Sunrise Senior Living policies to promote the highest degree of quality care and services to our residents.

RESPONSIBILITIES & QUALIFICATIONS
Essential Duties

Supporting our Mission, Principles of Service, and Core Values is fundamental to this role. The responsibilities include:

  • Identifying ongoing needs and services of residents to promote high-quality resident care.
  • Ensuring proper follow-up, evaluation, and documentation for residents with changes in health status.
  • Assisting the Resident Care Director (RCD) in conducting resident evaluations at move-in, 30 days post-move-in, semi-annually, and with any health changes.
  • Conducting monthly wellness visits for all residents.
  • Monitoring and following up with residents after falls, accidents, or incidents, and documenting as required.
  • Maintaining accurate and complete wellness files.
  • Communicating with residents’ families or responsible parties to ensure medical needs are met.
  • Contacting attending physicians as necessary or upon family request.
  • Obtaining and recording weights and vital signs monthly for each resident.
  • Maintaining medical supplies and emergency kits.
  • Providing clinical support and assistance to team members as needed.
  • Following infection control practices and standards.
Medication Programs
  • Monitoring medication and treatment profiles for accuracy and adherence to physician orders.
  • Transcribing physician orders into MAR/TAR accurately and promptly.
  • Performing monthly MAR/TAR audits.
  • Understanding and applying the Six Rights of Medication Pass.
  • Acting as liaison with pharmacy services.
  • Monitoring medication and treatment carts for organization and safety.
  • Conducting medication observations and skills checks as directed.
Quality Assurance and Regulatory Compliance
  • Participating in quality improvement initiatives and meetings.
  • Ensuring compliance with all relevant regulations and resident rights.
  • Reviewing physician reports and ordering medications/equipment as directed.
  • Implementing and monitoring documentation related to resident health and wellness.
  • Participating in engagement programs and team meetings.
  • Completing required training and certifications.
  • Performing other duties as assigned.

Experience and Qualifications

To perform this job successfully, an individual must:

  • Be a graduate of an approved nursing program.
  • Maintain a current LPN/LVN license.
  • Have at least one year of experience in relevant healthcare settings.
  • Demonstrate knowledge of nursing practices, regulations, and assessment skills.
  • Be proficient in computer skills, including Microsoft Office and Sunrise applications.
  • Be flexible to work weekends, evenings, and variable hours.
