About The Opportunity

This is a designated position. The successful candidate must self-identify as African Nova Scotian or Black Person of African Descent to be considered.

Reporting to the Health Services Manager, the Wellness Navigator supports community members in accessing health services, programs, and resources to promote health and wellness. The role involves providing guidance, information, and support, collaborating with an inter-professional team, and may require travel within the catchment area.

Qualifications
  • Bachelor's degree in Occupational Therapy, Social Work, Recreation Therapy, or related health discipline from an accredited institution.
  • Current registration with the relevant professional association.
  • At least three years of recent experience in mental health, primary health care, or community-based settings.
  • Experience in education, self-management support, and motivational interviewing.
  • Experience working with individuals with chronic conditions.
  • Preferred experience in partnership development and community engagement.
  • Knowledge of population health, health promotion, and behavior change principles.
  • Person-centered, family-focused, and culturally competent care approach.
  • Strong interpersonal, communication, and teamwork skills.
  • Flexibility, adaptability, and innovation in program planning and evaluation.
  • Proficiency in MS Office, email, and internet.
  • Annual CPR-AED and WHMIS certifications.
  • Valid Nova Scotia Driver’s License and reliable vehicle.
  • Additional languages, especially French, are an asset.

Hours and Compensation
  • Permanent full-time position; 75 hours biweekly.
  • Shifts may include evenings, weekends, and holidays.

Hourly rate: $36.58 - $47.35.
