Job Description

This position is responsible for providing a combination of unit clerical duties and direct patient care duties as needed on the designated Patient Care Unit. The incumbent ensures that accurate and complete information is completed to facilitate coordination and communication between the healthcare team. The incumbent performs clerical duties supporting the Patient Care Unit and provides assigned direct patient care duties under the supervision of the registered nurse.

Job Responsibilities

  1. Functions as a co-worker to the nurse and a key member of the nursing care team:
  • Organizes patient care assignment and sets priorities.
  • Alerts RN to changes in patient status (e.g., comfort, skin integrity, breathing pattern).
  • Documents patient care and status changes in hospital computer system and charts.
  • Communicates routine information to RN during shift, such as intake and output.
  1. Assists RN in providing for ADL needs of patients:
  • Prepares patients for meals, feeds, or assists with meals.
  • Provides between-meal nourishment and water.
  • Bathe or assist with bathing and oral hygiene.
  • Toilets patients as needed.
  • Maintains mobility with exercises and ambulation.
  • Grooms patients, including shaving as needed.
  • Participates in developing care plans with RN.
  1. Maintains patient care units in a neat and orderly condition:
  • Changes and makes beds.
  • Cleans and straightens bedside and over-bed tables.
  • Prepares rooms for new admissions, transfers, and post-operative patients.
  1. Assists Registered Nurse with procedures and treatments:
  • Records vital signs, fluid intake/output.
  • Performs EKGs, draws blood, obtains specimens.
  • Administers enemas, weighs patients, performs simple procedures.
  1. Performs related duties:
  • Assists patients with mobility, transports patients, maintains equipment and rooms.
  • Performs observation and supports QA/QI efforts.
  • Participates in continuing education and maintains medical records.
  1. Additional responsibilities include:
  • Notifying staff of new patients, screening calls, maintaining rosters and records.
  • Requesting maintenance and supplies, managing inventory.
  • Supporting patient rights, confidentiality, and participating in orientation and team activities.
  • Engaging in quality assurance activities and completing required documentation.

Job Qualifications

  • High School Diploma or equivalent, required.
  • Completion of Nurse Assistant course or equivalent, required OR prior experience as a Nursing Attendant or equivalent, required.
  • NYS Nurse Attendant certification preferred.
  • Effective communication skills, ability to follow instructions, work in diverse settings, and lift/move patients as needed.
  • BCLS certification required.
