The Transition Worker will also be actively involved in supporting youth to address various complex needs, including addiction and mental health, and teaching life skills that will allow the youth to transition successfully into independence.

  • Locating and supporting youth in maintaining safe and affordable housing.

  • Ability to facilitate relationships and connections, helping youth expand their personal support networks and find meaningful relationships.
  • Skills in crisis intervention, conflict resolution, critical thinking, problem-solving, and strong written and verbal communication.
  • Knowledge of community resources.
  • Current First Aid Certification and Suicide Intervention Training.
  • Current police check and Child Intervention Record Check (within the last six months).
  • Valid driver’s license, adequate insurance coverage (2 million dollars third-party liability), and own transportation.
