St. Joseph's Health Centre (SJHC) is seeking one casual Transition Planner for the Medicine program. The role focuses on delivering exceptional patient experiences and creating seamless transitions throughout the episode of care, driving the discharge plan, and providing continuity of information and compassion to patients, families, and care teams. The Transition Planner shapes expectations through high-quality communication, working closely with unit-based, multi-professional teams from admission to discharge. As part of a centralized Transition team, the Transition Planner collaborates with internal and external partners and contributes to knowledge translation of best practices across departments.

Qualifications

• Bachelor's degree in a health-related discipline; Master's preferred
• Registration in good standing with relevant professional college or regulatory body in Ontario
• 3-5 years recent discharge planning experience
• Strong knowledge of alternate levels of care, including home and community resources, long-term care, complex continuing care, rehabilitation programs, palliative care, and transitional care services
• Experience in complex discharge planning
• Excellent communication, negotiation, and interpersonal skills
• Knowledge in conflict resolution and creative problem solving
• Patient and family-centred transition planning and advocacy skills
• Organizational and coordination skills to manage multiple demands
• Ability to collaborate effectively with team members and community agencies
• Commitment to continuous learning and knowledge translation
