Job Summary

This posting will be used to fill 1 permanent, full-time position working 75 hours bi-weekly. It will establish a 12-month eligibility list which will be used to fill future vacancies. This job is based in Whitehorse, Yukon.

This position is within the Home Care branch of Continuing Care, offering services tailored to individual needs, including acute care, rehabilitation, and support for chronic and palliative conditions. Services are determined through personalized assessments to help clients remain in their homes, provide respite for caregivers, and promote independence and quality of life. Your Role

This position is responsible for performing assessments, evaluating care requirements, and assigning clients requiring Continuing Care Services (including home care and long-term care) to appropriate staff for client care navigation. You are an essential point of contact and collaborative partner for stakeholders such as the Yukon Hospital Corporation, to support client discharges to Continuing Care programs, while supporting an Aging in Place and Home First Philosophy.

Essential qualifications

  • Eligible for Registered Nurse registration with a Canadian regulatory authority
  • Recent experience in screening, intake, and coordinating services for clients with complex health conditions transferring to a long-term care/home care setting
  • Experience providing advanced nursing assessments within the scope of a Registered Nurse

Conditions of Employment

You must meet the following conditions before being hired, or within the specified time frame.

  • TB screening
  • RCMP Vulnerable Sector Check
  • WHMIS 2015
  • Valid class 5 driver’s licence
  • CPR Level C with AED (completed within the past 30 months)
  • Transportation of Dangerous Goods (completed within the past 30 months)
  • Current registration with the Yukon Registered Nurses Association
Job Requirements

This job requires you to perform, and/or be exposed to, the following:

  • Heavy lifting/bending/reaching during client transfer
  • Exposure to pets, allergens, infectious diseases, second-hand smoke and chemicals in cleaning supplies
  • Travel during adverse weather conditions
