The Transfer of Care Clinician partners with services within the Mental Health and Addictions Program to coordinate the Central Intake admission process. The role involves assessing clients’ needs, collaborating with referral sources, and ensuring clients are connected to the most appropriate services. The clinician maintains positive relationships with clients, families, and service providers, responding promptly to requests and concerns.

Hourly rate: $36.58 - $47.35
