About the Opportunity

Reporting to the Manager of Central Intake, the Transfer of Care Clinician collaborates with services within the Mental Health and Addictions Program. The role involves coordinating the Central Intake admission process to connect clients with appropriate services, assessing client needs, and maintaining positive relationships with clients, families, and referral sources.

About You

We are looking for candidates with:

  • Bachelor's degree in a health-related discipline (e.g., Nursing, Social Work, Occupational Therapy)
  • At least 3 years of clinical experience
  • Strong clinical assessment skills
  • Knowledge of mental illnesses and their impact on clients
  • Additional language skills, especially French, are an asset

Hours and Compensation

  • Full-time, permanent position; 75 hours bi-weekly

Hourly rate: $36.58 - $47.35
