About the Opportunity

the Transfer of Care Clinician works in partnership with services within the Mental Health and Addictions Program in accordance with established standards of professional practice and vision values and policies of Nova Scotia Health to coordinate all aspects of the Central Intake admission process to ensure clients / patients transferred / referred to Central Intake are connected to the most appropriate services and programs to meet their unique needs. The clinician works in collaboration with respective transfer / referral sources to assess the needs of clients / patients and in collaboration with the Transfer of Care team to determine what service will best address the needs of the client. The clinician establishes and maintains positive relationships with clients families transfer / referral sources and designated services responding to requests and concerns in a timely basis.

About You

We would love to hear from you if you have the following :

  • Bachelors degree in a health related discipline (I.E Nursing Social Work Occupational Therapy)
  • Must have Certification with the Provincial self-regulated health college / governing body
  • 3 years relatedexperience required
  • Competencies in other languages an asset French preferred

Hours

  • Long assignment full-time position; 75 hours bi-weekly
  • Assignment is approximately 12 months in length

Compensation and Benefits

36.58 - $47.35 Hourly
