What You’ll Be Doing
- Perform clinical reviews of referral information, prepare concise synopses and provide timely updates on referral statuses and program details to clients, referral sources and support networks
- Collect and organize required clinical documents, including discharge summaries, psychiatric assessments and lab results, to efficiently process complex referrals
- Input and update pre-admission information in the patient care system, follow up on missing documentation and ensure accurate record-keeping
- Partner with an interdisciplinary team to address unique patient needs, such as accommodations for service animals or physical limitations
- Respond to inquiries with professionalism, answer program-specific questions and recommend additional resources as appropriate
- Handle telephone lines, waitlists, emails and voicemails in collaboration with the Intake Services Team, ensuring responsiveness and efficiency
- Maintain and prioritize waitlists, allocate beds based on urgency and availability, and coordinate admissions or cancellations as needed
- Analyze referral and admission data, track trends and generate reports to provide actionable insights for leadership
- Leverage data and trends to propose pricing strategies, process improvements and sales tactics that align with organizational goals
What We’re Looking For
- Minimum 5 years’ experience in a healthcare related role such as nursing, case management or allied health
- Bachelor’s degree in a health-related discipline is preferred; diplomas in health-related disciplines will be considered
- Familiarity with mental health assessment tools (e.g., DSM-5 criteria, SCID) and ability to recognize signs and symptoms of various mental health conditions
- Proficiency in diverse interview techniques, with the ability to adapt methods based on patient needs and circumstances
- Skilled at accurately and succinctly documenting interviews, including patient responses and key observations
- Strong understanding of ethical and legal responsibilities, including maintaining confidentiality, obtaining informed consent and recognizing situations requiring mandatory reporting for safety
- Ability to maintain composure and manage personal emotions when discussing sensitive or distressing topics
- Exceptional attentiveness to verbal and non-verbal communication, ensuring a patient-focused approach
- Awareness of cultural differences and their influence on behaviour, communication and expressions of distress
- Strong commitment to prioritizing patient needs and delivering outstanding, compassionate service
- Comfortable using virtual meeting platforms, CRM systems and other relevant tools
- Ability to manage multiple priorities efficiently while maintaining high accuracy and attention to detail
- Experience using data analysis to identify trends, refine strategies and drive improved performance outcomes
