Job Summary
We are hiring dynamic, passionate Therapy Assistants (OTA/PTA) who want to be part of the transformational change in home and community care by providing care in a truly client-centered approach. You will be part of a diverse, collaborative care team providing services to clients in the community, focusing on holistic, client-centered goals across physical, cognitive, social, emotional, and spiritual domains. As a member of the interdisciplinary team, you will assist clients and families in reaching their care goals. As part of the SE Transitions team, you will be accountable for the care and services you provide and the outcomes your clients achieve.

Job Responsibilities:

  • Provide comprehensive care within an interdisciplinary team.
  • Support care delivery for clients, maintaining effective communication and collaboration.
  • Participate in team discussions and communicate client status effectively.
  • Assist clients in care activities as directed by Physiotherapists and/or Occupational Therapists.
  • Perform delegated acts/treatments within your role and policies.
  • Prioritize client safety and adhere to health policies and guidelines.

Requirements:

  • OTA/PTA Diploma from an accredited program.
  • Preferred minimum of one year relevant experience.
  • Experience in home/community care, acute care, chronic disease management, health promotion, or palliative care.
  • Community experience is an asset.
  • Current Vulnerable Sector Check.
  • Effective communication skills and good judgment.
  • Flexibility, organizational skills, and ability to work independently or in a team.
  • Valid driver’s license and vehicle, with ability to travel.
