Job summary

Woodstock Hospital Wellness Centre seeks a Registered Practical Nurse to assess health status, develop client-centered care plans and advocate for safe, quality outcomes. The role requires collaboration with the healthcare team and regular shifts including days, nights, weekends and holidays.

Strong communication, data reporting, and leadership skills are essential. Must hold RPN certification and CPR; MH/addictions experience is preferred to support policy development and safety initiatives.

Qualifications

• Current Certificate of Competence as an R.P.N.
• Current CPR Certificate.
• 3-5 years relevant mental health and addictions experience.
• Non-Violent Crisis Intervention course preferred.
• Applied Suicide Intervention Skills Training certification preferred.
• Trauma-Informed Care training certification preferred.

• Works with the client and family/caregivers to formulate a client centered plan of care.
• Advocates for clients to achieve optimal client outcomes within Expected Length of Stay.
• Utilizes time management skills to organize plan and provide care to clients.
• Works collaboratively with the healthcare team, promoting an environment of teamwork and collaboration.
• Participates in problem solving, planning, implementation and evaluation of Client safety and Quality Improvement initiatives.
• Expectation to attend work on a regular basis for all shifts including Days, Nights, Weekends and Holidays

Registered Practical Nurses are responsible for practicing within their scope to provide safe, competent, ethical, evidenced based client care using professional knowledge and critical thinking while meeting practice standards of the College of Nurses and the Regulated Health Professions Act.

The Registered Practical Nurse (RPN) located at the Wellness Centre will provide care under the Terms & Conditions outlined by the Ministry of Health for Homelessness and Addiction Recovery Treatment (HART) Hub care providers.

Essential Job Functions:
