Job summary

Extendicare Inc. is seeking a Registered Practical Nurse (RPN) to provide planned, patient‑centered nursing care under provincial standards. You will join an interprofessional team, develop and adjust resident care plans, and document changes promptly to ensure quality outcomes.

The role requires RPN registration, experience in senior care, and BCLS/First Aid certifications. You will support education for unregulated providers and coordinate care with physicians and the nursing team.

Qualifications

• Current registration as a Registered Practical Nurse in the applicable province or state.
• Experience working in a senior’s environment and with unregulated care providers preferred.
• Education and experience in gerontology, rehabilitation, community/complex/long‑term care is an asset.
• Strong written and oral communication, critical thinking, care coordination, time management and customer‑focus skills.
• Demonstrated sound clinical knowledge.
• Computer proficiency and experience with Microsoft Office applications.
• Current Basic Cardiac Life Support and First Aid certification required.
• Current VPS.

Job description

Mission

Reporting to the Director of Care or Director of Health and Wellness, a Registered Practical Nurse (RPN) provides planned, patient‑oriented, goal‑directed nursing care in accordance with provincial standards of care.

Key Responsibilities

• Work as a member of the interprofessional team within Extendicare’s Professional Practice Model, applying a person-centered approach to care.
• Complete and ensure resident clinical care assessments, care/service plans, and documentation are accurate, up-to-date, and changes are communicated as required.
• Implement and evaluate the effectiveness of care/service plans and make changes as needed.
• Assist with scheduling resident care conferences and participate in conferences where applicable.
• Work closely with DOC/DHW to facilitate resident move-ins, move-outs and transfers.
• Provide direct resident care, medication/treatment administration, and document care in compliance with regulatory and Extendicare Resident Care standards and policies.
• Process orders, safely keep drugs and narcotics, and administer them in compliance with regulatory requirements, scope of practice and Extendicare policies.
• Facilitate and provide resident/family education.
• Plan, coordinate, manage and evaluate residents’ care plans with the resident/substitute decision maker, physicians, nursing and interdisciplinary team, and contracted service providers.
• Coordinate nursing team resident care assignments to ensure efficient, timely delivery of services.
• Respond to and manage significant changes in resident condition or medical emergencies; provide emergency first aid.
• Teach, delegate to, and direct unregulated care providers.
• Respond to resident/family concerns and ensure appropriate action is taken, or concern is communicated to DOC/DHW/Executive Director and other departments.
• Assist DOC/DHW in the coordination, implementation and monitoring of Infection Prevention and Control, Health and Safety, Clinical Programs and Quality/Safety Plans for the Nursing Department.
• Uphold and promote the organization’s values and philosophy, particularly regarding ethics, morality, and integrity as set out in Extendicare’s Code of Conduct.
• Complete all other duties as assigned.
