Job summary

Vancouver Coastal Health is seeking a Nurse Case Manager to support a Richmond community client population through a client- and family-centered care model. You will coordinate care across the interdisciplinary team, apply chronic disease management principles, and liaise with physicians and community agencies to achieve optimal client outcomes.

RNs or RPNs must be registered with BCCNM with at least 3 years in case management for geriatric or chronic conditions, and hold a valid BC driver's

Qualifications

• Current practicing registration with BCCNM as RN or RPN.
• Minimum 3 years of case management experience with geriatric or chronically ill adults.
• Valid BC Driver's License; local travel may be required.

Responsibilities

• Coordinate and implement clinical interventions for clients with complex care needs.
• Liaise with physicians, nurses and community agencies to ensure continuity of care.
• Assess clients, identify problems, and adjust care plans in collaboration with the team.
• Provide client and caregiver education based on nursing theory.
• Act as a clinical resource for the team and contribute to evidence-based practice.

Job description

Come work as a Nurse Case Manager with Vancouver Coastal Health (VCH) in the Richmond Community! Within the context of a client and family/care giver centred care model and in accordance with the British Columbia College of Nurses and Midwives (BCCNM) standards of professional practice and Code of Ethics for registered nurses or registered psychiatric nurses, and the established vision and values of the organization, manages a select population of eligible clients who are chronically ill or have long term physical disabilities. Using chronic disease management and self-care management approaches, conducts assessments, identifies problems, addresses issues or variances from the plan of care in collaboration with the appropriate physician, interdisciplinary team member and/or community agency. Within a multidisciplinary team context, provides continuity of care across the continuum by liaising with team members, coordinating and implementing clinical interventions, evaluating client outcomes based on client goals for care, coordinating ongoing care and supporting transition. Acts as a clinical resource for the client and their care giver and the multidisciplinary team by providing information based on nursing theory and practice related to clients with complex health care needs. Consults, confers, and collaborates with multidisciplinary team members, physicians, educational resources and other health care professionals to achieve excellence in client-centred education and research.

The hours of work including days off and work area may be subject to change consistent with operational requirements and the provision of the Collective Agreement and applicable statutes.

As per Ministry of Health policy, all health care workers working in publicly-funded health care facilities are required to report their past receipt of certain vaccines or history of certain infections. Collecting these records will allow for offering of any missing vaccines, and for appropriate actions to be taken in the event of any future exposure to a communicable disease or during outbreaks. Education & Experience

• Current practicing registration as a Registered Nurse or a Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).
• Three (3) years' recent related experience in case management working with geriatric clients/adults with chronic, acute illness and/or acquired brain injuries or an equivalent combination of education, training and experience.
• Valid BC Driver's License. Local area travel requires the use of personal vehicle.

Knowledge & Abilities

• Comprehensive knowledge of case management practice within a client and family/care giver centred care model for clients with complex health care needs.

• Demonstrated skill in physical assessment and chronic disease management.
• Demonstrated competency in psychomotor skills required within the designated scope of nursing practice.
• Comprehensive knowledge of British Columbia College of Nurses and Midwives (BCCNM) standards of practice and guidelines for clinical practitioners.
• Demonstrated ability to work independently and set priorities for care for a caseload of clients.
• Comprehensive knowledge of other health disciplines and their role in client care.
• Comprehensive knowledge of other health and community resources related to the care of clients and care givers.
• Demonstrated ability to work collaboratively as a member of an interdisciplinary team.
• Comprehensive knowledge of adult education principles, methods and tools.
