Job description

As a team lead under the supervision of the Partnerships Coordinator, you will play a key role in guiding and supporting the PLM intervention team. You will ensure service quality and efficiency while actively contributing to the program’s development.

Your Responsibilities:

  1. Train and support the team, ensuring goals are met and guidelines are followed.
  2. Assist intervention workers in complex and urgent situations (crises, deaths, etc.).
  3. Collaborate with external partners and participate in coordination meetings.
  4. Work directly with beneficiaries: welcoming, assessing, planning actions, and providing personalized follow-up.
  5. Facilitate access to housing, health, and psychosocial services to enhance client autonomy.
  6. Oversee the planning and organization of activities stemming from PLM according to established procedures and the values of Welcome Hall Mission (WHM).
  7. Continuously assess and ensure that client needs are met, and contribute to development efforts to address these needs meaningfully.
  8. Attend various meetings (team leads with clinical coordination, team leads and PLM coordination) and ensure relevant information is shared with the PLM-WHM team.
  9. Collaborate with coordination to ensure smooth operations and synergy within the team.
  10. Manage team case files and renew OMHM documentation.
  11. Participate in developing and implementing solutions to address challenges within the team.
  12. Perform other related tasks.
