Job summary

St. Joseph’s Health Centre Guelph is seeking a Registered Nurse for Complex Continuing Care on a temporary part-time basis. You will assess, treat and promote health, while supporting staff and coordinating with an inter-professional team to meet patient goals.

Responsibilities include leading care plans, educating other health professionals, and ensuring safety and adherence to policies. A strong nursing background and relevant Ontario registration are required.

Qualifications

  • Registered Nurse in good standing with the College of Nurses of Ontario.
  • Strong conflict resolution and interpersonal skills.
  • Ability to lead and educate other health care providers.
  • Knowledge of Occupational Health and Safety and Infection Control practices.

Responsibilities

  • Care for patients across supportive, therapeutic, palliative and rehabilitative modalities.
  • Supervise and direct RPNs and PSWs on the unit.
  • Lead planning and evaluation of care plans with the inter-professional team.
  • Ensure adherence to SJHCG policies and standards.
  • Act as a preceptor to students and novice nurses.

Job Summary

The Registered Nurse is an integralmember of the health centre care team and participates in the assessmenttreatment, and provision of care and health promotion for all patientsthrough supportive, therapeutic, palliative and rehabilitative means whileadhering to the standards mandated by the College of Nurses of Ontario. Responsible for providing care to patients,as well as supervision and direction to staff such as Registered PracticalNurses and Personal Support Workers.The RN supports the mission, vision and values of SJHCG, as well asworking towards established goals and objectives. The RN is also responsible for followingall policies, processes and procedures of SJHCG.

  • Meets a wide range of nursing care needs of clientsregardless of complexity and predictability, including health teaching.
  • Meets immediate and anticipated long-term clientneeds, drawing from a comprehensive assessment and range of options.
  • Manages multiple nursing interventionssimultaneously in rapidly changing situations
  • Directs plans of care for highly complex clients.
  • Collaborates with client to evaluate overall goalachievement and modifies plan of care
  • Identifies and anticipates a multiplicity ofoutcomes and modifies plan of care in collaboration with client.
  • Recognizes, analyzes and interprets deviations frompredicted client response(s); modifies plan of care autonomously.
  • Consults with other health care team members about abroad range of client needs.
  • Acts as a resource for RPNs to meet client needs.
  • Designs, coordinates and implements health programs.
  • Assumes role of leader within inter-professionalteam.
  • Provides leadership through formal and informalroles.
  • Acts as a preceptor to students and novice nurses.
  • Directs unregulated care providers, as appropriate.
  • Leads team effort to develop plans of care toachieve identified client goals when overall care requirements are morecomplex.
  • Makes decisions about and allocates resources atprogram/unit level.
  • Critically evaluates theoretical and research-basedapproaches for application to practice
  • Appraises the value of evidence, incorporatesresearch into practice, develops research questions and participates onresearch teams.
  • Integrates theoretical and research-based approachesto design care and implement change.
  • Contributes to a transparent culture of client andstaff safety by adhering to and abiding by patient and staff safety policiesand procedures set by SJHCG.
  • The Program Team Leader is an additional resourcethat can be accessed for consultation for care decisions, or if there isconflicting views on how to move forward. The program team leader is responsible for the overall function of the program during their shift, and has decision making authority in the absence of the Program Manager. All nurses are collectively responsible for the effective function of their floor during their shift.
Qualifications & Experience
  • Registered Nurse in goodstanding with the College of Nurses of Ontario.
  • Strong conflict resolutionskills when dealing with staff, supervisors, physicians, patients and family.
  • Demonstrated written andverbal communication skills, and excellent interpersonal and leadershipskills.
  • Strong analytical andcritical thinking skills.
  • Ability to exercise goodjudgement in recognizing situations that may require immediate attention ofappropriate person/people.
  • Ability to provideeducational opportunities for other health care providers and health carestudents.
  • Knowledge of and adherenceto Occupational Health and Safety and Infection Control Practices, College ofNurses Standards of Practice, and Health Centre policy and procedures.
  • Strive to continuouslyprovide a safe and supportive environment for patients, staff, physicians,and volunteers.
  • Dedicated to professionalgrowth & development.
  • Excellent organization andprioritization skills.
  • Ability to workindependently and effectively communicate with other members of the healthcare team.
