Job Description

The Western York Region Ontario Health Team (WYR OHT) is a partnership of local health care providers committed to better health and experiences for patients and providers in Vaughan, Richmond Hill, King, and surrounding areas. Our vision is to improve community health by creating an interconnected system of care across the continuum, from prenatal care to end of life and bereavement.

Role Overview

The System Planner reports to the Director of the WYR OHT and is accountable to the WYR OHT Collaboration Council. The role involves building strong relationships with internal and external partners, employing leadership and best practices to plan and implement the OHT, and fostering a culture of collaboration among partners. The incumbent will support initiatives to advance integrated care across sectors including Primary Care, Acute Care, Home and Community Care, Public Health, and Community Support Services.

Key Responsibilities

  • Manage workload effectively in a fast-paced environment.
  • Address and resolve issues respectfully and productively.
  • Use sound professional judgment and best practice guidelines.
  • Contribute to a safe, inclusive environment by complying with safety policies.
  • Apply project management methodologies to plan and manage projects.
  • Develop and coordinate project management materials such as project plans, milestone maps, and status reports.
  • Lead and facilitate project meetings, develop reports, presentations, and statistics.
  • Evaluate project milestones to identify lessons learned and best practices.
  • Ensure project deliverables are met on time, within scope and budget.
  • Develop and implement project management tools for OHT members.
  • Manage interdependent planning work streams, timelines, and deliverables.
  • Mentor and support Project Managers and Engagement Specialists.
  • Prepare and submit grant/funding applications, coordinate reporting, and support evaluation of initiatives.
  • Support development of collaborative care and partnership opportunities.
  • Manage stakeholder maps, lists, and contacts.

Minimum Qualifications

  • Master’s degree in Public, Business, Healthcare Administration, or equivalent experience.
  • 1-3 years healthcare or public sector experience.
  • Membership in a regulated health professional college preferred.
  • PMP designation preferred.
