System Navigator (Black Children and Youth)
1 FTE Contract to March 31, 2026

Thunder Bay Site

One of a network of provincial community health centres, the NorWest Community Health Centres is a community-based, non-profit organization funded by Ontario Health, emphasizing access to primary health care and prevention programs and services.

We are currently seeking a 1 FTE System Navigator, as a valuable member of the Community Health Centres’ team who will ensure a range of health and social services aimed at improving Black health through the provision of guidance, assistance and supports for clients we serve to navigate the healthcare, public services and social service system through our health care delivery sites.

The position has a competitive salary, plus 4% vacation pay and the Health Care of Ontario Pension Plan is an optional benefit.

QUALIFICATIONS:

  • Bachelor's degree in a related field (e.g., social work, health sciences, community health).
  • Two to three years’ experience in health and social services, case management, or community outreach.
  • Ability to effectively engage clients in the assessment and planning with regards to their needs.
  • Demonstrated ability to facilitate and advocate for services to support clients in their care.
  • Ability to work both independently and collaboratively with an interdisciplinary team.
  • Strong writing and documentation skills.
  • A strong understanding of the social determinants of health framework.
  • Strong understanding and knowledge of health services and community supports.
  • Ability to work flexible hours.
  • Awareness and sensitivity to socio-economic and cultural issues.
  • Awareness of and sensitivity to health issues particularly within populations who face barriers to accessing care (under housed, living with mental health challenges or diagnoses, current or past substance use, complex health histories, 2SLGBTQIA+, newcomers, non-insured)
  • Valid driver’s license, access to a vehicle and ability to travel to other service locations.
