Job summary

Homeward Trust Edmonton is seeking a Supportive Housing Program Coordinator to provide culturally safe, client-centred support to Indigenous individuals and families living in supportive housing.

The coordinator will work with tenants to achieve housing stability, address barriers related to mental health and addictions, reconnect with culture, kin and community, and collaborate with partners to deliver housing and support services.

Qualifications

• Minimum three years in frontline case management or related housing services.
• Diploma or degree in Social Work or related field preferred.
• Knowledge of Indigenous history and trauma-informed practices.
• Experience with Housing First approaches is an asset.
• Familiarity with ETO or similar case management systems is an asset.

The Supportive Housing Program Coordinator provides culturally safe, client-centred support to Indigenous individuals and families living in supportive housing. This role supports tenants in achieving housing stability, strengthening family relationships, addressing barriers related to mental health and addictions, and reconnecting with culture, kin, and community. The Coordinator works collaboratively with community partners and service providers to help tenants achieve long-term wellness and independence.

This position provides on-site support and requires regular evening and weekend shifts to support program delivery. The Coordinator reports to the Team Lead, Indigenous Program Supports, and works closely with Homeward Trust staff, community agencies, and service providers. The successful candidate may also be assigned to the Housing First team to support Indigenous clients in securing and maintaining housing.

Key Responsibilities

• Provide culturally safe, trauma-informed, and client-centred case management to Indigenous individuals and families.
• Complete needs assessments and develop individualized support plans.
• Support tenants in achieving housing stability and maintaining successful tenancies.
• Connect clients to community resources, income supports, healthcare providers, traditional helpers, Elders, and cultural programming.
• Provide outreach services in clients' homes and community settings.
• Promote informed choice and deliver services using a harm reduction and strengths-based approach.
• Support family reunification, cultural reconnection, and community integration where appropriate.
• Assist clients with transportation when required.
• Maintain accurate client documentation and enter required information into the Efforts to Outcomes (ETO) database.
• Participate in program evaluation and reporting requirements.
• Collaborate with Homeward Trust staff, landlords, service providers, and community partners to coordinate supports.
• When assigned, support Indigenous individuals and families in accessing Housing First or other Homeward Trust-funded housing programs.
• Perform other duties as assigned.

Education & Experience

• Minimum three (3) years of front-line experience in case management, supportive housing, community outreach, mental health, addictions, Indigenous services, or housing programs supporting vulnerable populations.
• Diploma or degree in Social Work, Human Services, Community Planning, Psychology, Sociology, Indigenous Studies, Human Geography, or a related field is preferred. Equivalent combinations of education and experience will be considered.
• Demonstrated knowledge of Indigenous history, cultures, traditions, and the impacts of colonization, residential schools, systemic discrimination, and intergenerational trauma.
• Experience applying trauma-informed, client-centred, harm reduction, and evidence-based approaches to service delivery.
• Experience supporting vulnerable populations experiencing housing instability.
• Experience with Housing First practices and philosophies is considered an asset.
• Experience using the Efforts to Outcomes (ETO) database or a similar case management system is considered an asset.

Knowledge, Skills & Abilities

• Strong understanding of Indigenous cultures, traditions, values, and community supports, with familiarity working alongside First Nations, Métis, and Inuit communities in Alberta.
• Ability to provide culturally safe, flexible, and client-centred supports that recognize the importance of relationships, family, kinship, ceremony, and healing.
• Experience supporting individuals and families in rebuilding relationships, reconnecting with culture, and overcoming the impacts of intergenerational trauma.
• Knowledge of housing resources, income support programs, and community services available in Edmonton.
• Excellent communication and relationship-building skills with clients, community agencies, and internal stakeholders.
• Strong assessment, problem-solving, advocacy, and case management skills.
• Ability to work independently, exercise sound judgment, and maintain confidentiality.
• Strong organizational and time management skills, with the ability to manage competing priorities in a fast-paced environment.
• Compassionate, respectful, and collaborative approach to supporting clients.

Working Conditions

• Tuesday to Saturday, 12:00 p.m. to 8:00 p.m.
• Flexibility to work evenings and weekends as operationally required.
• Valid Class 5 driver's licence and ability to travel within the City of Edmonton.
• Successful completion of a Criminal Record Check, including a Vulnerable Sector Check.
