The Person-Centered Support Worker (PCSW) actively engages and participates in all aspects of the experience of children, youth, and/or adults. The PCSW provides individualized care and support to individuals with complex needs, including behavioral support needs, developmental disabilities, mental health challenges, and other related needs. The role involves creating a safe, positive, supportive, and therapeutic environment, and implementing person-centered care plans tailored to the individual's needs.

Services include:

  1. Providing care and support to individuals with complex needs.
  2. Participating in the creation and support of person-centered care plans.
  3. Developing and implementing activity plans.
  4. Teaching life skills such as self-care, cooking, cleaning, budgeting, and positive interactions.
  5. Supporting activities of daily living, including bathing, dressing, grooming, feeding, and toileting.
  6. Performing health-related tasks and monitoring health conditions.
  7. Assisting with medication management as prescribed.
  8. Resolving conflicts and responding to crises using behavioral support strategies.
  9. Communicating with the care team to ensure continuity and quality of care.
