JOB TITLE: Health For Two Support Worker

REPORTS TO: Program Supervisor

HOURS: 37.5 hours/week

Job Purpose

Health For Two is a prenatal program that provides support, information, and resources throughout pregnancy and until 2 months post-partum to women who require extra support due to health and/or social issues. The Health For Two Support Worker reports to the Program Supervisor at Boyle Street Community Services and works primarily offsite in the community providing services to pregnant women and support to community agencies. Health for Two is delivered in all Public Health Centres in the Edmonton Zone and in over 45 different community agencies and is funded through the Public Health Agency of Canada.

Key Areas Of Responsibilities
  1. Provide case management services to clients on the Health for Two Program, working closely with partner agencies, Public Health Centre nurses, and/or Hf2 Network Nurses.
  2. Build relationships and rapport with families that are referred to the program.
  3. Assess client’s needs in order to provide assistance, support, and advocacy on issues including but not limited to: housing, food security, financial and child protection services, medical, legal, educational opportunities.
  4. Build and maintain positive relationships with agency and Alberta Health Services staff and work collaboratively on referrals, case management and consultation activities.
  5. Provide emotional support and social support to clients, including cultural supports.
  6. Maintain up-to-date knowledge on various community and government programs in order to provide relevant information and make appropriate referrals.
  7. Attend case conferences as necessary.
  8. Maintain accurate comprehensive documentation regarding all interactions with clients in accordance with agency standards.
  9. Participate in agency responsibilities as time allows including but not limited to: morning meeting, committees, reflections, safety and security monitoring.
  10. Attend relevant Hf2 meetings including Network Meetings, Network Team meetings, Hf2 Team Meetings, and Support Worker/Coordinator meetings.
  11. Work collaboratively with Network Coordinator to meet the needs of the agency partners, health centres, and the network itself. This includes agency visits, consultation, in-services, and capacity building.
Qualifications

Education:

Degree or equivalent. Candidates without a degree but who have post-secondary education and relevant and related experience will be considered.

  7. Capable of supervising and motivating practicum students and/or volunteers.
  8. Able to plan and make decisions in a timely, organized and appropriate manner.
  9. Openness and intentionality regarding issues facing traditionally marginalized populations.
  10. Knowledge of best practices in harm reduction.
  11. Must be calm and confident in chaotic or crisis situations.
Experience And Other Requirements
  1. At least 2 years’ experience delivering community based services.
  2. Experience networking and developing relationships with a range of partners and stakeholders.
  3. Excellent knowledge and understanding of social determinants of health including poverty, mental health and addictions, trauma etc.
  4. Comfortable working with a diverse population including immigrants and refugees, homeless, isolated, substance using etc.
  5. Knowledge of historical issues related to Canada’s history of colonization of Indigenous peoples; additionally, must be open to learning about, and participating in, traditional Ceremony.
  6. Good working knowledge of Microsoft Office (Word, Excel, Outlook).
  7. Must have valid Class 5 Driver’s license, reliable vehicle, and appropriate vehicle insurance coverage.
  8. Significant travel will be required including client transportation, so adequate insurance is required.
  9. Criminal Record and Child Intervention Checks (results will be reviewed on a case by case basis).
