A local health service provider is seeking Support, Time & Recovery Workers in Bodmin. Support, Time & recovery workers (ST&R) are required as part of the exciting new Integrated Non-Custodial Services (INCS) contract, which has been awarded to DPT for the next seven years starting in April 2025.
The INCS contract brings together Criminal Justice, Liaison & Diversion , Reconnect and the Mental Health Treatment Requirement (MHTRs) in order to deliver care and support for individuals to address their health care needs and offending behaviour from custody diversion through to release back to the community.
Reconnect offers care after custody support that seeks to improve the continuity of care of individuals with identified health needs, including mental health, learning disability, substance misuse, physical health, neurodivergent needs and other psychosocial vulnerabilities, leaving prison by supporting their transition to community-based health and support services through liaison, advocacy, signposting and support
Main duties of the job
As part of your role you will hold a caseload supporting offenders as they are released from prison with any identified health need. This will involve assessing them in prison prior to their release, identifying their needs and collaboratively drawing up a care plan to meet their needs. The service can work with people up to 6 months post release but this will be determined by individuals needs.
Multi agency work will be essential as the role requires close working with prison teams, including physical and mental health services, substance misuse services, community based health care services, probation, and housing organisations.
