Responsibilities

• Create visual daily schedules and adapt activities to group and individual needs.
• Monitor and track person-centered goals with deadlines.
• Assist persons-served in building a network of community supports and involve families.
• Coordinate support strategies and monthly reporting on activities and their therapeutic value.
• One weekend a month designated on-call for home program.
• Collaborate with funding bodies, RCMP and other officials; ensure trauma-informed, therapeutic interventions.
• Collaborate with Support Liaison team on agency-wide projects and events.
• Attend meetings, appointments, case-conferences and training as delegated.
• Participate in monthly supervisions and staff meetings.
• Liaise with family members for holistic service delivery.
• Support onboarding with Agency tours.
• Complete other delegated tasks at Leadership discretion.

Program:

The Nova Program is a supported living environment with two operating locations in the community of Grande Prairie. The Nova Program is committed to equipping our persons-served with the necessary tools and skills to develop the life skills necessary to live as independently as possible through their day-to-day lives. The Nova Support Home is intended for long-term support, supporting adults into their senior years. Programming is highly individualized, allowing for a high quality, person-centered support model.

Role:

The role of the Support Liaison is to develop and implement both group and individualized programming initiatives for persons-served. The Support Liaison role is dynamic, and requires flexibility, priority management skills, and the ability to work collaboratively with multiple teams. The Support Liaison must anticipate and respond to both operational and service delivery needs of the program.

The Support Liaison will report directly to their Program Manager and indirectly to their division's Service Enhancement Coordinator (Executive Leadership). The Support Liaison will work in collaboration with their home program team as well as the Support Liaison team to create and facilitate initiatives that will drive and inspire personal growth and development for persons-served. Using both trauma-informed and person-centered approaches, the Support Liaison will build meaningful relationships with persons-served and will assist in teaching life and independent living skills to the individuals served in a cooperative environment.

This position includes variable hours and shifts, including but not limited to potential day shifts, awake night shifts, and sleep shifts. In addition to this, Support Liaisons will be responsible for one (1) weekend on-call per month in their designated home program.

Responsibilities:

• Use creativity and flexibility to create a visual daily schedule based on program and individual needs, focusing time on appropriate group activities and individualized programming

• Monitor and track individuals’ person-centered goals, and work with program team to ensure deadlines are adhered to and recorded

• Assist persons-served in creating a network of community supports and involve relevant agencies and family members where appropriate

• Coordinate and communicate all support strategies and community networking plans with their Program Manager, including monthly reporting on scheduled activities and their therapeutic value to persons served

• One weekend a month designated on-call for home program

• Professional collaboration with program stakeholders, including funding bodies, RCMP and other law officials, representatives of the justice system, and community members

• Champion programming initiatives to Agency members to ensure all staff are supporting persons-served using trauma informed, therapeutic interventions

• Collaborate with Support Liaison team to implement Agency-wide programming projects and events

• Attend all meetings, appointments, case-conferences and training as delegated by their Program Manager

• Participate in monthly supervisions with respective supervisors and attend monthly staff meetings

• Liaise and collaborate with family members or natural supports of persons-served to ensure a holistic approach to service delivery

• Support employee onboarding process within the Agency by facilitating Agency tours

• Complete all other delegated tasks at the discretion of Leadership
